RN - Hospice & Homecare Nurse

Deliver compassionate, individualized patient care following tailored care plans designed to meet hospice and homecare needs across all age groups, from newborns to the elderly. Ensure safety principles are consistently applied and collaborate closely with physicians, RN case managers, and other healthcare professionals to optimize patient outcomes.

Key Responsibilities:

• Conduct hospice and home health admission assessments, including OASIS evaluations, and develop initial patient care plans.
• Supervise Home Health Aide services in accordance with organizational policies.
• Participate in on-call rotations to provide 24-hour nursing support for hospice, homecare, and private duty patients.
• Maintain accurate and timely documentation as per agency standards.
• Foster positive relationships with patients, families, colleagues, physicians, and referral sources.
• Promote patient independence by setting care goals and involving patients and families in discussions about medical care, medications, and self-care techniques.
• Observe and assess patients' physical, mental, emotional, and spiritual well-being, collaborating with the care team as needed.
• Adhere strictly to HIPAA regulations to protect patient health information.

Education:

• Graduate of an accredited nursing program is required.
• Bachelor of Science in Nursing (BSN) is strongly preferred.

Experience:

• Several years of clinical nursing experience.
• Previous hospice and/or home health experience is preferred.
• Certification as a Hospice and Palliative Care Nurse is a plus.

Requirements:

• Current Registered Nurse licensure in the relevant state.
• BLS certification required within three months of hire.
• Valid driver's license and auto insurance meeting state requirements.
• Proficiency with MS Office and strong keyboarding skills.
