Territory Fairmont, Avondale, Camp Washington, Mt Washington, Cherry Grove, Goshen, Loveland, Midland, Pleasant Plain, West Chester, Miamiville, Sharonville, Batavia, Georgetown, Owensville, Sardinia, Lebanon, Mason, Morrow, Sabina areas. HOW YOU'LL MAKE A DIFFERENCE As an RN, you will provide one‑on‑one patient care, work at the top of your license, and be part of an interdisciplinary team focused on compassionate quality care and positive outcomes. You will interact with patients' families while caring for patients and experience the rewarding privilege of being part of every step of their recovery journey. HOW YOU'LL WORK You will deliver comprehensive skilled care as ordered by the attending physician within the specified geographical territory. You will work autonomously and manage your time efficiently, handling all duties within your scope of practice. MAJOR AREAS OF RESPONSIBILITY Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change. Administer medication as prescribed by the physician and help decrease re‑hospitalizations by prioritizing visits for high‑risk patients. Manage multi‑disciplinary care, promoting continuity of care with admissions, transfers, and discharges. Educate patients and families on disease processes using teach‑back methods to ensure understanding. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided. Understand and follow agency policies, procedures, rules, and regulations, and communicate changes in schedule or availability to schedulers or supervisors. Perform other duties as assigned. Attention to detail and observant, following directions accurately. REQUIREMENTS Registered Nurse with a current license in the state of employment. Minimum one year of experience as an RN in an acute care setting. Home care experience preferred. Valid driver’s license and auto insurance in your name as a driver.
