Direct Support ProfessionalAssist participant with needs identified in the Individualized Life Plan such as, self-care, toileting, showering, tooth brushing, grooming, dressing and ensure proper hygieneDistribute and collect linen for laundry service in accordance with procedure.Assist participant in attaining skills needed for daily living and socialization according to their abilitiesSafely accompany participant to community resources (stores, places of worship, recreation, and health facilities, etc.)Assist with meal preparation and serving of meals.Responsible for cleaning of serving utensils and pots.Complete Incident Reports as appropriate.Complete room checks as needed.Update logbook to foster greater and meaningful communication across shifts as needed.Demonstrate the ability to work as part of a teamInteract with clients to promote a friendly supportive but professional atmosphereAll other duties as assigned by the Shift Supervisor or other supervisory staffAttending staff meetings and in-service trainingParticipate in meetings as requestedOther related tasks that may be assignedJob Requirements:High School Diploma or Equivalency with two years of experience preferredVerbal and written communication skills essential to perform function of position.Ability to speak/write EnglishSCIP-R Certification/TrainingCPR/First Aid
