Internal Medicine - Geriatrics Physician

Schedule:

• 3 days per week (preferred Tues Thurs)
  
• 7:00 AM 5:00 PM (10-hour shifts)
  
• No night call (covered by Hospitalists)

Practice Highlights:

• Mix of SNF, LTC, transitional and post-acute care
  
• Average patient volume: patients/day
  
• 99-bed department
  
• Dedicated APP support
  
• Admissions required (2/day)
  
• Rounding on assigned patients
  
• Supervision of APP required
  
• EMR: MediTech

Clinical Responsibilities Include:

• SNF admissions, H&P completion, and recertification visits
  
• Long-term care medical management
  
• Chronic disease and preventative care
  
• Medication management and care plan oversight
  
• Acute condition management and transition of care coordination
  
• Participation in interdisciplinary meetings and goals-of-care discussions
  
• Management of geriatric and medically complex patients, including dementia and frailty

Requirements:

• BC/BE Internal Medicine or Family Medicine
  
• Experience in Skilled Nursing, Long-Term Care, Transitional Care, or Post-Acute Medicine preferred
  
• MT License or IMLC required
  
• BLS & ACLS required
  
• Temporary privileges available
  
• Credentialing timeframe: Approximately 120 days
