Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Virginia Physical Therapy license required to start the assignment.
• Must have at least 12 months of home health experience within the past 3 years.
• Experience with HCHB (HomeCare HomeBase) EMR.
• BLS certification from AHA or ARC may be required.

Preferred Qualifications

• Previous travel or per diem home health experience preferred.

Assignment Details

• Shift hours: Monday - Friday, 8:00 AM - 4:30 PM.
• Weekend availability as needed.
• Area of coverage: Suffolk and Portsmouth.

Impacting Patient Care

Provide one-on-one home health physical therapy to patients in their residences. Use HCHB to document skilled interventions, progress toward goals, and discharge planning. Coordinate care with interdisciplinary teams to support safe transitions and improved functional outcomes.
