LinkJobs works with large health care systems, small clinics and everything in between, and we are currently searching for the right person for the following position:Manage the transitional and chronic care needs of homebound patients including:Assessment and evaluation of current conditionObtain past medical historyReview symptomsConduct general physical examinationsReview and reconcile medicationsScreen for depressionScreen for alcohol and drug abuseIdentify diagnoses to support care management and treatment planningRecommend appropriate measures, including medication prescriptions and skilled care needsEfficiently communicate assessment findings to the office/supervising physicianEducate patients on disease processes, prevention, medications, and complianceQualifications:Current Nurse Practitioner license in the state of Georgia with no sanctions or restrictionsMinimum of 1 year of clinical experienceClinical background in Adult, Family, or Geriatric specialties preferredExperience using Electronic Health Records (EHR)Strong communication skillsComfortable using smartphone and emailReliable transportation requiredAbility to work independentlyHome care or home visiting experience preferredMust maintain active individual malpractice insurance coverageWillingness to spend 30 minutes to 1.5 hours with patients in their homes, which may include pets and/or household tobacco usePosition Type:1099 Independent Contractor
