RN Case Manager Hospice

Job Summary:

The Hospice Registered Nurse (RN) is responsible for admitting patients to hospice services, conducting comprehensive assessments of patients and families physical, psychosocial, environmental, safety, and developmental needs. The RN develops individualized care plans, documents observations and interventions, coordinates care with a multidisciplinary team, and participates in quality assessment and performance improvement programs. The role includes conducting follow-up visits as needed, primarily serving patients in San Francisco, San Mateo, and Santa Clara counties. A minimum of one year of hospice nursing experience is required. Responsibilities:

• Admit patients to hospice services and perform initial and ongoing assessments.
• Evaluate patients and families physical, psychosocial, environmental, safety, and developmental needs.
• Develop and update individualized plans of care based on assessments.
• Document patient observations, interventions, and care plans accurately.
• Coordinate patient care with an interdisciplinary hospice team.
• Participate in hospice program quality assessment and performance improvement activities.
• Conduct follow-up visits and reassess patient needs as required.
• Communicate effectively with patients, families, and healthcare providers to ensure optimal care delivery.

Required Skills & Certifications:

• Active Registered Nurse (RN) license in the state of practice.
• Minimum of one year hospice nursing experience.
• Strong clinical assessment and care planning skills.
• Excellent documentation and communication abilities.
• Experience working collaboratively within an interdisciplinary team.
• Bachelor s degree in Nursing or related field preferred.
• Driver s license, car insurance, and reliable automobile for field work.

Preferred Skills & Certifications:

• Experience with hospice quality assessment and performance improvement programs.
• Additional certifications related to hospice or palliative care nursing.
• Familiarity with electronic medical record systems.

Special Considerations:

• Work involves travel within San Francisco, San Mateo, and Santa Clara counties.
• May require flexibility in scheduling to meet patient needs and follow-up visits.
• Role requires strong interpersonal skills to support patients and families facing serious illness and end-of-life care.

• Work schedule may vary based on patient needs and geographic territory.
• Typical shifts may include standard business hours with occasional need for follow-up visits outside normal hours.
