Occupational Therapist

Position Summary:
The Occupational Therapist (OT) provides in-home therapy services to help patients improve independence with daily activities. This role focuses on evaluating patient needs, creating treatment plans, and delivering care that supports recovery and functional mobility.

Key Responsibilities:
Evaluate patients' physical and functional abilities in their home environment
Develop and implement individualized treatment plans
Provide therapy to improve daily living skills, mobility, and independence
Educate patients and caregivers on exercises, safety, and home modifications
Monitor patient progress and adjust treatment plans as needed
Complete timely and accurate documentation of all patient visits
Collaborate with physicians and care team members
Travel to patient homes within the assigned area
Maintain patient confidentiality and follow all healthcare regulations (including HIPAA)
Meet productivity expectations and manage daily schedule effectively

Required Qualifications:
Active Occupational Therapist (OT) license
Graduate of an accredited Occupational Therapy program
Ability to work independently in a home health setting
Strong communication and clinical decision-making skills
Valid driver's license and reliable transportation

Preferred Qualifications:
Experience in home health or home care
Experience with Homecare Homebase (HCHB) or similar EMR systems
Strong time management and organizational skills
