Cardiac Surgery/Cardiogenic Shock- Advanced Practice Clinician - Rotating Shifts - Abington

Job Details Position: Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) within the cardiac surgery and cardiogenic shock service at Abington Memorial Hospital, Jefferson Health. Provides comprehensive inpatient care including pre‑ and post‑operative cardiac surgery management and care of critically ill patients with cardiogenic shock, including those requiring mechanical circulatory support (ECMO, Impella, IABP). ICU experience is a plus. Work shift: rotating. Primary location: 1200 Old York Road, Abington, Pennsylvania, United States of America. Responsibilities Perform comprehensive assessment (history & physical) and document in the medical record. Order and collect data using appropriate assessment techniques and diagnostic procedures. Serve as first responder for nursing and house staff during patient status changes, orders, and procedural or resuscitative interventions. Assess risks associated with care of acute and complex chronically ill patients, including medication side effects, immobility, impaired nutrition, fluid & electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures. Collaborate with interdisciplinary team to develop diagnoses of acute and complex chronic conditions. Manage diagnostic tests through ordering, interpreting, performing, and supervising. Formulate differential diagnoses by priority. Diagnose complications and order appropriate interventions. Identify expected outcomes and formulate a plan of care addressing complex acute and chronic health care needs. Utilize evidence‑based practice guidelines and protocols in an individualized, dynamic plan of care applicable across the continuum. Implement and modify plan of care. Prescribe diagnostic strategies and therapeutic interventions. Perform advanced procedures consistent with privileges and competency validation. Maintain ongoing communication and collaboration with interdisciplinary team, patient, family, and staff to promote continuity of care across inpatient and outpatient settings. Document progress via daily progress notes, transfer, discharge summary, clinic notes, procedure notes, acute event notes, death summaries, etc., timely and meeting compliance standards. Demonstrate professional practice behaviors including preceptor/mentoring, education, instruction of graduate students, research engagement, participation in advanced practice grand rounds, specialty‑specific meetings, CEU maintenance, and membership in professional organizations. Qualifications Master’s degree. National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant. BLS certification. Relevant clinical experience (e.g., ICU, cardiac surgery) preferred.
