Position & Responsibilities: Plan, implement, and evaluate individualized music therapy sessions for hospice patients based on their unique needs, preferences, and goals. Utilize a variety of music therapy techniques, including live music interventions, guided imagery, songwriting, and improvisation, to address physical, emotional, cognitive, and spiritual aspects of care. Assess patients' responses to music therapy interventions and modify treatment plans as needed to optimize therapeutic outcomes. Collaborate with the interdisciplinary team to integrate music therapy into the plan of care, ensuring coordination and continuity of services for patients receiving hospice care. Provide education and support to patients, families, and caregivers on the benefits of music therapy and techniques for using music as a coping mechanism during end-of-life transitions. Document patient assessments, treatment plans, progress notes, and outcomes in accordance with regulatory requirements and professional standards. Participate in interdisciplinary team meetings, case conferences, and in-service trainings to enhance collaboration and professional development. Board-certified music therapist (MT-BC) credential from the Certification Board for Music Therapists (CBMT). Minimum of 1 year of experience as a music therapist, with specific experience in hospice, palliative care, or healthcare settings preferred. Proficiency in playing a variety of musical instruments and vocal techniques. Strong interpersonal and communication skills, with the ability to establish rapport and build therapeutic relationships with patients and families. Compassionate and empathetic approach to patient care, with sensitivity to the unique needs of individuals facing end-of-life challenges. Ability to work effectively as part of a multidisciplinary team, collaborating with nurses, social workers, chaplains, and other healthcare professionals. Flexible schedule and willingness to travel to patients' homes, nursing facilities, and hospice care centers as needed. #J-18808-Ljbffr
