Case Manager II (Charity & Disaster Case Management) - Northeast Region Full time, New York Job Description End-to-End 1115 Workflow Mastery Develop and maintain thorough working knowledge of the full 1115 Waiver program lifecycle, from participant intake, screening, navigation through service delivery and billing closure. Serve as the internal subject-matter expert on how each process stage connects to the next, including handoff protocols between teams; including A. Process Navigation & Exception Management B. Program Eligibility & Compliance Oversight C. Invoice Oversight & Clarification D. Site Coordinator Support & Accountability E. Voucher & Reimbursement Coordination Collaborate with the Volunteer Coordinator and team leaders to develop, package, and refine operational workflows for volunteer engagement within the 1115 program. Contribute to onboarding materials, training protocols, and role-clarity documentation to support a smooth and scalable operation. Coordinate cross-functional logistics to ensure program activities are wellsupported by volunteer capacity. Job Requirements Understands how individual processes connect to the broader program architecture. Tracks details, follows through, and maintains accountability without being prompted. Works effectively across teams – volunteers, staff, and finance – adapting communication style as needed. Comfortable navigating complex or evolving program guidelines and making sound judgments.
