WesleyLife in Urbandale, IA is seeking an Admissions Nurse for our Hospice team. You will travel to CHLs and homes to see hospice clients and families, perform comprehensive assessments, and initiate individualized care plans with ongoing updates. Responsibilities include head-to-toe assessments, updating the plan of care for re-certifications, coordinating orders with physicians, and counseling patients and families to support comfort and self-sufficiency. #J-18808-Ljbffr WesleyLife
