A well-established multidisciplinary healthcare practice in Fort Collins is seeking a motivated Nurse Practitioner to join our growing team. We specialize in physical medicine, rehabilitation, regenerative medicine, and medical weight loss. Responsibilities:

• Conduct comprehensive new patient evaluations and examinations
• Perform patient re-evaluations and progress assessments
• Develop and manage individualized treatment plans
• Prescribe physical therapy and rehabilitative services
• Prescribe and dispense durable medical equipment (DME)
• Perform trigger point injections
• Perform joint injections, including PRP (Platelet-Rich Plasma) procedures
• Administer and manage regenerative medicine treatments, including human cellular tissue products
• Coordinate care with chiropractic and rehabilitation providers
• Manage medical weight loss programs
• Maintain accurate and timely clinical documentation
• Educate patients regarding treatment options and ongoing care

Qualifications Required:

• Active Nurse Practitioner license in the State of Colorado
• Strong clinical assessment and diagnostic skills
• Excellent communication and interpersonal skills
• Ability to work effectively within a multidisciplinary team
