Clinical Lead NP: Home-Based Chronic Care

Strive Health

Strive Health is seeking a Clinical Lead Nurse Practitioner to guide care for patients with CHF, CKD/ESKD and other chronic conditions. You will work across home, clinic, and dialysis settings using telehealth and analytics to deliver longitudinal, patient-centered care.
