Come join our team!The RN, Care Manager will evaluate members' needs and requirements to achieve and/or maintain an optimal wellness state. This position will guide members and their families toward interaction with resources appropriate for the care and well-being of members. The RN, Care Manager will collaborate with a multi-disciplinary team employing a variety of strategies, approaches, and techniques to manage members' physical, environmental, and psychosocial health issues. The RN, Care Manager will primarily perform fieldwork outside of the office setting. The incumbent will travel to locations within the VIVA HEALTH service area through a reliable means of transportation insured in accordance with Company policy.
