MDS Coordinator

Cape Heritage Rehabilitation & Health Care Center in Sandwich is hiring an MDS Coordinator! Shift is 8a-4p with weekly pay. Can be an LPN or RN. $42 to $48 an hour, depending on license. Cape Heritage is a 120-bed skilled nursing facility. As the MDS Coordinator, you help plan, organize, and direct the MDS process, involving overseeing resident care plans through clinical assessment, review of residents' medical history, personal interviews, and completion of MDS reports. Experience & Education: Must possess, as a minimum, a nursing degree from an accredited school of nursing, college, or university. Must possess an active and unencumbered license to practice as an LPN or RN in Massachusetts. Duties & Responsibilities: Coordination of MDS process. Oversees ADL training for the facility and staff. Issues and delivers denial notices timely and appropriately. Complete and transmit all CMS approved item sets (MDS). Must be knowledgeable of and follow current CMS regulatory guidelines as described in RAI Manual. Complies with facility privacy policies and procedures and protects residents’ individual health information. Maintains Medicare meeting minutes per Medicare program agreement. Issues and delivers Medicare denial letters per CMS regulations. Assures appropriate management of residents’ Medicare/Insurance benefits. Maintains adequate systems to ensure appropriate documents are sufficient to support billed services. Specific Requirements: Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such an action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must be able to coordinate MDS systems, resident assessment, and care plans for each resident timely. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
