Required:
Active and unrestricted Connecticut RN License or Compact RN License.
Associate's or Bachelor's Degree in Nursing.
Previous Hospice Nursing Experience.
Experience using Homecare Homebase (HCHB).
Strong case management and care coordination skills.
Ability to work independently while providing compassionate patient-centered care.
Excellent communication and interdisciplinary collaboration skills.
