Senior Case Manager - Elderly Services (Field & On-Site)

The Mexican American Opportunity Foundation is seeking a Case Worker to assess needs of elderly clients and develop service plans to support independence and prevent hospitalization. This on-site role requires travel to client homes and community settings as needed.
