Remote Case Manager, Home Care & Community Health (Ohio)

CareStar, Inc. is seeking a Case Manager for the Ohio Home Care program covering Miami, Greene, Clark, and Montgomery Counties. The role blends remote work with on-site field visits to assess needs, develop care plans, and connect clients to essential services. The candidate will conduct eligibility evaluations, coordinate across health and social services, and support transitions in care. A Bachelor's degree and relevant licensure are required, with prior home care experience preferred.
