Why This Role Stands Out Predictable schedule: Monday–Friday, 8:00 AM–4:00 PM No nights, weekends, or on-call Hybrid flexibility: 2 days working remotely Meaningful patient relationships: Spend time understanding the person—not just the diagnosis Autonomy: Take ownership of your caseload and care coordination Real impact: Help older adults remain healthy, safe, and independent at home Collaborative environment: Work alongside a supportive multidisciplinary team What You’ll Do Conduct comprehensive in-home assessments Develop and manage individualized care plans Manage a caseload of older adults with complex medical and social needs Coordinate services among physicians, specialists, caregivers, and community resources Educate patients and families on care needs, resources, and available services Monitor progress and identify changes in health or care needs Maintain accurate clinical documentation and ensure compliance Collaborate with an interdisciplinary team to improve patient outcomes What We’re Looking For Active Massachusetts RN license 3+ years of nursing experience; 5+ years preferred Experience in case management, home care, community health, geriatrics, or care coordination is a plus Strong clinical judgment and critical-thinking skills Excellent communication and organizational abilities Comfortable working independently while collaborating with a team Experience with EMR systems and Microsoft Office Reliable transportation for local travel A Better Kind of Nursing Career This is an opportunity to use your nursing expertise in a way that's relationship-driven, proactive, and focused on long-term outcomes . #J-18808-Ljbffr Newbridge Recruiting Group LLC
