RN Case Manager I

We are seeking a Registered Nurse Case Manager to provide comprehensive care for members with complex needs, ensuring quality and cost-effective treatment. Key Responsibilities Include Developing, implementing, and evaluating individualized care plans aligned with physician orders. Delivering competent, quality patient care in the home setting. Managing an assigned caseload and ensuring ongoing assessment and updates to care plans. Communicating effectively with staff, providers, and members' families. Providing patient education and participating in team knowledge sharing activities. Performing additional duties as assigned. Qualifications And Skills Licensed RN in the Commonwealth of MA, with a minimum of 3-5 years' clinical home care experience. Bachelor of Science in Nursing preferred. Proficient in Microsoft Office and case management data systems. Ability to travel locally frequently. Spanish speaking and COVID vaccination preferred.
