RN Case Manager – Hybrid
Hybrid | 2 Days Remote + 3 Days Field/Office
Monday–Friday | 8:00 AM–4:00 PM
No nights No weekends No on-call

We're seeking an RN Case Manager to support older adults with complex healthcare needs, helping them navigate services, coordinate care, and remain safe and independent in their homes.
You'll have the opportunity to build lasting relationships with patients and families while enjoying a consistent weekday schedule and hybrid flexibility.

- Conduct comprehensive in-home assessments
- Develop and manage individualized care plans
- Manage a caseload of older adults with complex medical and social needs
- Coordinate services among physicians, specialists, caregivers, and community resources
- Educate patients and families on care needs, resources, and available services
- Monitor progress and identify changes in health or care needs
- Maintain accurate clinical documentation and ensure compliance
- Collaborate with an interdisciplinary team to improve patient outcomes
- Active Massachusetts RN license
- 3+ years of nursing experience; 5+ years preferred
- Experience in case management, home care, community health, geriatrics, or care coordination is a plus
- Strong clinical judgment and critical-thinking skills
- Excellent communication and organizational abilities
- Comfortable working independently while collaborating with a team
- Experience with EMR systems and Microsoft Office
- Reliable transportation for local travel
