RN-Case Manager

This role involves assessing, developing, and evaluating patients' discharge planning needs in collaboration with an interdisciplinary team. Responsibilities ensuring compliance with discharge planning standards, identifying and addressing gaps related to inpatient and observation stays, assessing high-risk patients for readmission, and developing comprehensive discharge plans. The nurse may also manage utilization review, emergency department case management, admission processes, and liaise with post-acute care providers such as skilled nursing facilities, rehab, and home health. Qualifications graduate of an accredited nursing program, with at least three years of acute care experience. Must hold a valid RN license in the applicable state(s). Physical ability to perform lifting, walking, and other movements required for patient care. Skills and attributes adherence to professional nursing standards, use of evidence-based practices, alertness, safety awareness, effective communication, and teamwork.
