Medical Assistant/Patient Population Management Coordinator

Job Summary and Responsibilities As our Medical Assistant, you'll keep the day running smoothly and efficiently for your patients and clinical team. Every day, you will take the lead on proactively preparing patient charts, ensuring all information is obtained and up-to-date for those seen in our facility. You can expect to have multiple patients daily depending on the number of physicians are on-site. Your success in this role comes from your enthusiasm to help others, work efficiently, and your empathy for our patients. Obtain all required information for the provider prior to the patient's appointment Prepare the patient prior to the provider entering the treatment room. Keep accurate documentation in the patient's chart pertaining to the appointment or any interaction with a patient within her/his scope of service, (i.e., daily chart preparation for visit, medication refills or response to written order given by the provider or phone messages). Interview individuals and obtain comprehensive and accurate information, prioritize incoming telephone calls appropriately. Communicate effectively in oral and written form. Communicates effectively with all members of the health care team. Job Requirements Minimum Qualifications: High school diploma (or equivalent) required Possesses a Certificate to practice as a Medical Assistant (MA Diploma or Certificate from accredited program required) Has the ability to read, write and speak English in order to perform duties of the position Current BLS required (must be certified through the American Heart Association) Where You'll Work Dignity Health Medical Foundation established in 1993, is a California nonprofit public benefit corporation with care centers throughout California.
