Role Description The Case Management Nurse is a full-time, on-site role based in Lubbock, TX. This role coordinates patient care across hospice and home health services, including assessing clinical needs, developing individualized care plans, and facilitating transitions between care settings. Day-to-day responsibilities include collaborating with physicians and interdisciplinary teams, monitoring patient progress, managing utilization of services, and ensuring appropriate discharge planning and follow-up. The Case Management Nurse communicates regularly with patients, families, and caregivers to provide education, support, and advocacy, while maintaining accurate documentation and adhering to regulatory and organizational standards. Qualifications Candidates should possess strong clinical skills in Nursing and Medicine, with the ability to assess patient needs and collaborate on treatment plans. Candidates should possess expertise in Case Management and Utilization Management to coordinate services, optimize care pathways, and support effective resource use. Candidates should possess proficiency in Discharge Planning, including organizing transitions of care, arranging follow-up services, and educating patients and families. Active, unrestricted RN license in Texas (or eligibility for licensure) and prior experience in hospice, home health, or acute care settings are preferred. Strong communication, critical thinking, and organizational skills, as well as the ability to work collaboratively with interdisciplinary teams and diverse patient populations. Knowledge of applicable regulations, documentation standards, and electronic health record systems; case management certification is a plus. #J-18808-Ljbffr
