Social Worker

As a Social Worker at Potomac Valley Rehabilitation and Healthcare Center, you will play a vital role in supporting our residents and their families in achieving their physical, emotional, and social well-being. Our 165-bed facility is well-regarded for its high-quality rehabilitation, skilled nursing, palliative care, and behavioral health programs. We are seeking a compassionate and experienced Social Worker to join our multidisciplinary team.
About the Role:
Provide social work services to our residents, including assessments, treatment planning, counseling, and education
Develop and implement individualized care plans in collaboration with our interdisciplinary team
Identify and address the social and emotional needs of our residents and their families
Facilitate communication and coordination with our medical staff, rehabilitation therapists, and other healthcare professionals
Participate in care conferences and other meetings to ensure that our residents receive comprehensive and coordinated care
Responsibilities:
Conduct initial and ongoing assessments of our residents' social and emotional needs
Develop and update individualized care plans to address identified needs and goals
Provide counseling, support, and education to our residents and their families
Collaborate with our medical staff and rehabilitation therapists to ensure comprehensive care
Facilitate communication between our residents, their families, and our interdisciplinary team
Participate in quality improvement initiatives to enhance our services and outcomes
Requirements:
Bachelor's degree in Social Work or related field
Minimum of two years of experience in social work, preferably in a healthcare setting
Licensure as a Social Worker in the State of Maryland (or ability to obtain)
