Strong background in psychosocial assessment, crisis intervention, behavioral health support, discharge planning, and community resource coordination for maternal-infant populations.
Knowledge of postpartum mental health, trauma-informed care, social determinants of health, and maternal-infant care coordination.
Ability to assess patient needs and prioritize urgent, emergent, and routine psychosocial and discharge planning needs in a fast-paced environment.
Flexible, resourceful, and able to work independently while maintaining a calm, professional, and patient-centered approach.
Proficiency in electronic medical record systems, including Epic.
2–5 years of experience in psychiatric social work or counseling, with a focus on maternal-child health, behavioral health, crisis intervention, discharge planning, or care coordination.
