Job Description

• Facilitate communication between the patient and the interdisciplinary team.
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
• Utilize state-of-the-art technology to document skilled services provided.

Qualifications

• Licensed Practical Nurse with valid CT LPN license.
• One year of LPN experience; homecare experience preferred.
• Valid driver's license and reliable transportation required.
• Computer proficiency required.

Regular standard hours per week: 40. Schedule: Full-time (40 hours). Shift details: Shoreline towns including Stonington, Groton, Waterford, East Lyme, Old Lyme, and Old Saybrook, Mystic, Noank and North Stonington.
