Social Services Designee

Job Overview Position: Social Services Designee at Webster House Pay: $31.66 - $36.00 per hour Schedule: Part Time, 32 Hours Weekly, Monday – Thursday, 9 am – 5 pm Responsibilities Provide quality customer service to residents, families, coworkers, and vendors. Interview new residents to assess psychosocial needs and collect medical, psychological, and social information. Provide individual or family counseling regarding plans to meet needs and mobilize internal and external resources. Refer residents to community health resources and other organizations. Coordinate discharge planning and ensure timely documentation of social service interventions and assessments in the resident chart. Compile records and prepare reports such as theft and loss reports. Integrate social services with other elements of the resident’s care plan through staff meetings and/or conferences. Coordinate with nursing department to meet residents’ optical, dental, and audiological needs and arrange transportation. Attend resident care meetings, rehabilitation and utilization review, quality assurance, and department head meetings. Promote safety by providing equipment, personal safety gear, and appropriate safety instructions, and by following safe work habits to prevent injuries. Qualifications Essential: Social Services Designee. Educational Requirement: Bachelor’s Degree in Social Work or related field. Experience: Minimum 6 months of social services in a skilled nursing environment. Nonessential: 1 year of leadership or supervisory experience.
