Discharge Planner

The Discharge Planner is responsible for coordinating the discharge process for all CSU clients, post discharge follow-up, maintaining cooperative relations with community agencies and service providers, and meeting with families to develop and implement appropriate discharge plans for after care.

This position requires a Bachelor's Degree in a Behavioral Healthcare field with a minimum of 2 years of mental health related experience. Master's Degree preferred.

Provides and documents timely and thoroughly treatment services, assessments, interventions, and aftercare plans.

Facilitates the exploration of continuing care issues that include: returning home, post program placement, participation in self-help groups, partial hospitalization, and intensive out-patient programs.

Arranges appropriate post discharge referrals.

Engages with collateral contacts and providers regarding client's discharge needs.

Participates in treatment teams to provide collateral information for discharge planning.

Enters information into CFBHN's SharePoint system as required.

Computer Literacy.

Completes all training requirements in a timely manner.

Completes all other duties and special projects as assigned.
