Key Responsibilities\n

• Conduct thorough assessments of patients’ mental health status, utilizing standardized tools such as the Diagnostic and Statistical Manual of Mental Disorders to create personalized treatment plans that cater to individual cultural and social needs.

• Continuously evaluate and monitor patient progress, adjusting interventions and strategies as necessary to ensure the most effective support for their mental health conditions.

• Prepare and maintain accurate treatment records and clinical information promptly, aligning with regulatory standards and company procedures to ensure quality of care.

• Collaborate closely with patients and their families to design comprehensive treatment plans that respond to their specific needs and desired outcomes, ensuring they feel involved in their care process.

• Act as a liaison between patients and external treatment providers, coordinating referral options and fostering partnerships to enhance patient support systems.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active and unrestricted Washington State license as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n

Community Uplifters LLC offers a competitive salary range of USD 90,000 to 105,000 annually, commensurate with experience and qualifications.
