About the Role:

Join Vantage Hospice as a Hospice RN Case Manager in the vibrant Sugarland and SW Houston areas. In this rewarding role, you'll provide compassionate care and support to patients and their families during critical times.

Responsibilities:

• Conduct comprehensive assessments of patients' needs and develop individualized care plans.
• Coordinate and manage patient care across multidisciplinary teams.
• Provide nursing care, education, and emotional support to patients and families.
• Monitor patient progress and adjust care plans as necessary.
• Maintain accurate and up-to-date patient records and documentation.
• Participate in case conferences and collaborate with healthcare providers.
• Ensure compliance with hospice regulations and quality standards.
• Act as a liaison between patients, families, and the healthcare team.

Requirements:

• Current Registered Nurse (RN) license in the state of Texas.
• Previous experience in hospice, palliative care, or home health preferred.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate demeanor with a focus on patient-centered care.
• Basic Life Support (BLS) certification required.
• Knowledge of hospice regulations and best practices.
• Proficient in electronic medical records and documentation.
