Position Summary: Under the supervision of the Program Director, the Care Coordinator is responsible for the coordination of care for all ROWI clients, including individualized service such as orientation, transportation, doctor visits, and scheduling. Duties and Responsibilities: Ensures programs and services are implemented at the highest standards and clients receive the highest level of care. Responsible for client supervision and ensuring they attend designated groups throughout programming. Facilitate assigned groups and activities as designated by the Facility Manager. Conduct client orientation for all admits on day of admission or assign to another staff if not available. Includes UA screening, PHQ9, and other assessments as directed.
