Key Responsibilities

• Conduct thorough assessments to diagnose acute and chronic medical conditions in residents, ensuring timely and effective care.
• Develop, implement, and manage individualized care plans in collaboration with a healthcare team that includes physicians, nurses, and therapists.
• Utilize advanced clinical skills to order, interpret, and follow up on diagnostic tests while prescribing appropriate medications and treatment regimens.
• Educate residents and their families about disease management, medication adherence, and post-treatment requirements, ensuring they feel informed and empowered.
• Facilitate interdisciplinary rounds, contributing to collaborative discussions regarding resident care strategies and outcomes.
• Maintain precise and detailed electronic medical records to ensure compliance, accountability, and continuity of care.
• Engage in quality improvement initiatives that enhance care practices and contribute to the facility's overall mission.
• Stay abreast of the latest healthcare best practices and regulatory requirements to ensure adherence to the highest standards of care.

Patient Population / Client Focus

This role primarily focuses on managing an older adult population, many of whom may have complex health issues requiring thoughtful management. You will work with residents who may be managing chronic conditions such as diabetes, hypertension, and dementia, as well as assisting with acute changes in health status. Your personalized attention will significantly impact their quality of life and engage them in their care plans.

Qualifications

• Active Nurse Practitioner certification; current state license to practice nursing.
• Bachelor's degree in nursing required; a Master's degree in nursing from an accredited program is preferred.
• Strong clinical expertise with a minimum of 3-5 years of experience in a healthcare or hospital setting, ideally with elder care.
• Excellent communication, collaboration, critical thinking, and problem-solving skills are essential to thrive in this role.

Preferred Experience

Experience specifically within an assisted living or long-term care environment is highly desirable. Familiarity with electronic health record (EHR) systems for precision in documentation and communication is advantageous. Compensation and Benefits

The position offers a competitive salary range of USD 120,000 – 220,000 per year, dependent on experience and qualifications.
