Nurse Practitioner or Physician Assistant

HouseCalls Advanced Practice Clinician (NP/PA) $30,000 Sign-On Bonus or $40,000 Student Loan Repayment Optum Home & Community Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its HouseCalls team. This role focuses on conducting annual in-home health assessments for Medicare Advantage and other health plan members. Why This Role Stands Out Flexible scheduling with the ability to work between 8 AM–8 PM No on-call No holidays Opportunity to achieve strong work-life balance What You'll Do Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing as appropriate Identify and document diagnoses Communicate findings and care gaps to primary care providers Educate patients on conditions, medications, and health management Address social determinants of health and connect members with resources Recognize and respond to urgent or emergent situations Required Qualifications NP: ANCC or AANP certification (Family, Adult, Geriatric, or AGPCNP) PA: NCCPA certification Active state licensure (or ability to obtain before start) Valid driver\'s license and reliable transportation Ability to travel to patients\' homes and navigate various home environments Preferred Qualifications At least 1 year of clinical experience (family practice, geriatrics, home health, etc.) Experience working with Medicare or geriatric populations Comfortable using EMR systems and technology Ability to carry equipment up to 30 lbs.
