Physical Therapist (PT)

The Physical Therapist (PT) provides direct rehabilitation care within a skilled nursing facility, focusing on long-term support and short-term rehabilitation for diverse patient populations. Key Responsibilities Develop, implement, and modify individualized treatment plans based on physician referrals and patient needs. Supervise and evaluate therapy assistants, aides, and students. Document patient progress accurately and timely using therapy software. Collaborate with interdisciplinary teams to deliver patient-centered care. Assist in patient transfers and ensure safety protocols are followed. Participate in community health initiatives and uphold compliance with laws and regulations. Qualifications & Skills Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT). Valid license or registration in the applicable state, maintained in good standing. Preferred one year of post-acute care experience. CPR certification (must remain current). Proficiency in Microsoft Office and excellent communication skills. Ability to make independent decisions, demonstrate professionalism, and maintain confidentiality.
