Responsibilities

• Deliver skilled nursing care to patients in their homes according to individualized care plans
• Perform comprehensive patient assessments and ongoing evaluations
• Administer medications, IV therapies, wound care, injections, and other prescribed treatments
• Monitor patient progress and identify changes in condition that require intervention
• Develop, implement, and update individualized care plans in collaboration with physicians and the interdisciplinary care team
• Educate patients and caregivers on disease management, medications, treatments, safety, and preventive care
• Coordinate services with therapists, social workers, case managers, physicians, and other healthcare professionals
• Maintain accurate, timely, and complete documentation in the electronic medical record (EMR)
• Ensure compliance with home health regulations, infection prevention standards, and patient safety guidelines
• Promote patient independence while delivering compassionate, high-quality care

Requirements

• Active California Registered Nurse (RN) License
• Minimum 2 years of recent Home Health nursing experience
• Current BLS Certification required
• Valid driver's license and reliable transportation for patient visits
• Experience with OASIS documentation preferred
• Knowledge of chronic disease management, wound care, medication administration, and patient education
• Strong assessment, clinical decision-making, and care coordination skills
• Experience using electronic medical record (EMR) systems
• Excellent communication, organization, and time management skills
• Commitment to delivering outstanding patient-centered home healthcare
