The Access Case Manager ensures a seamless intake process for clients across EOSIS Treatment Centers, providing consistent support from initial contact through admission. This role builds relationships with clients, referral partners, and families while facilitating intake decisions, coordinating services, and supporting access to appropriate levels of care across multiple program locations 

Pay : $58,000 - $62,000 Annually  
Shift:  Full-Time

• Conduct brief biopsychosocial assessments to determine program fit and level of care 

  • Establish rapport quickly with clients and community stakeholders 

  • Apply knowledge of person-centered care, systems theory, and the 12 core functions 

• Assist prospective clients in accessing services 

  • Navigate community resources including financial, housing, and support services 

  • Determine eligibility based on client information, records, and collateral data 

  • Coordinate service initiation and admissions planning across locations 

• Manage communication related to admissions and referrals 

  • Verify funding sources and determine client financial responsibility 

  • Facilitate smooth transitions into programming across EOSIS sites, including transportation coordination 

  • Address treatment barriers (social, financial, logistical) 

• Monitor and manage waitlists across programs 

  • Evaluate system status to support intake approvals and placement decisions 

  • Ensure readiness for admission and continuity of care across multiple sites 

• Complete and maintain accurate documentation 

  • Document client needs and maintain compliance with program requirements 

  • Perform data entry and maintain daily census logs across assigned programs 

• Support care team transitions 

  • Provide warm hand-offs to internal clinical teams at the appropriate service location 

• Build and maintain referral relationships 

  • Strengthen partnerships with community providers 

  • Represent EOSIS programs with professionalism and customer service focus 

Required Skills REQUIRED QUALIFICATIONS  

Education:  

• Bachelor’s degree in Human Services, Business, or related field 

Experience:  

• At least 1 year of experience in a co-occurring (chemical dependency and mental health) program 

• Minimum of 4 years of case management experience OR applicable licensure 

Skills, Knowledge, and Abilities:  

• Strong organizational skills and attention to detail 

• Effective verbal and interpersonal communication skills 

• Ability to explain complex concepts in a clear, simple manner 

• Ability to work with diverse populations across cultural and socioeconomic backgrounds 

• Strong problem-solving and adaptability in a fast-paced environment 

• Proficiency in computer systems, documentation, and communication tools 

• Ability to work independently with minimal supervision 

PREFERRED QUALIFICATIONS  

• 3+ years of experience in a chemical dependency treatment setting 

• 2–3 years of related healthcare or case management experience 

• Licensure (LSW, LGSW, LISW, LICSW, or LADC) 

• Experience in sales, marketing, or business development 

• CPR certification (or ability to obtain prior to hire) 

• Valid Minnesota driver’s license

• Criminal Background Check: Nationwide, federal, state, and multi-county (up to 7 years) 

• Drug and Alcohol Testing: Must pass pre-employment screening; subject to random and post-incident testing if in a safety-sensitive role

• Motor Vehicle Driving Record review if necessary 

WORKING CONDITIONS / WORKING REQUIREMENTS  

Physical Requirements:  

• Sitting: Up to 6 hours continuously 

• Walking: Frequent 

• Travel: Up to 15% between EOSIS program locations (as needed) 

• Lifting, standing, bending, squatting, reaching: Minimal 

• Stair climbing: Minimal (varies by assigned location) 

Work Environment:  

• Work is performed across multiple EOSIS treatment sites or intake settings 

• Fast-paced healthcare environment with varying program structures 

• Frequent interaction with clients experiencing substance use and mental health challenges 

• Requires flexibility to support multiple programs, locations, and changing priorities
