Human Services Eligibility Case Manager (Human Services Case Manager 1) Location: 647 1st Ave N, St. Petersburg, FL 33701. Schedule: Monday – Friday; 8:00 a.m. – 5:00 p.m. Hybrid schedule including office, outposts, and some remote work. Within our Community Connections Division, we partner with the Florida Department of Health to deliver the Pinellas County Health Program, providing prevention-focused healthcare, mental health and substance use services, specialty care referrals, and case management for eligible residents. Our Case Managers work in various outposts throughout the county to increase accessibility to our services. Employees in this class exercise considerable independent judgement in obtaining health and social services for clients. A high degree of public contact is exercised, requiring professionalism, tact and poise. Screen and interview applicants in person and over the phone, and responds to questions regarding availability of, and eligibility for benefits. Process applications to determine eligibility for medical assistance, advises applicants of needed documentation, and refers ineligible applicants to alternate community resources for assistance. Provide information and referral services to clients. Work with community partners and other agencies to assist clients in accessing services and programs. Answer phone calls from the general public to provide information and resources. Maintains an active caseload for eligibility and short-term case management. Enters accurate and timely notes, and scans documentation and forms in appropriate databases. Applies policies and procedures of the department's eligibility determination process. Perform data entry and record keeping. Prepare and prints medical ID cards and issues to clients. Attend meetings and trainings. Perform other related job duties as assigned. What Do You Need? Experience: Technical and professional experience in social/human services case management or related field. Degree : Social Work, Psychology, Sociology, or a related field. 5 years of experience as described above; or Associate's degree and three (3) years of experience as described above; or Bachelor’s degree and one (1) year of experience as described above; or Master’s or higher-level degree as described above. An equivalent combination of education, training, and/or experience. A valid Florida Driver License, in good standing, will be required to travel to offsite locations and various trainings and meetings. Assignment to work a variety of work schedules including compulsory work periods in special, emergency, and/or disaster situations. Ability and willingness to travel throughout Pinellas County. Participation in the Pinellas County Employee Disaster Preparedness & Response operations. Remote Work Policy: This position requires employees to live within a 60-mile radius of Pinellas County and maintain the ability to report to the assigned worksite as needed. If you relocate outside the required 60-mile radius, you must receive advance approval from the County Administrator. Failure to meet this requirement may affect your continued eligibility for the position. Highly Desirable Knowledge of services offered by the County and other local community services agencies.
