HIV/AIDS Care Coordinator & Case Manager

Metro Healthy Communities in the Town of Florida, New York, is seeking a Medical Case Manager to provide essential case management services to individuals impacted by HIV/AIDS. The role involves developing individualized service plans and coordinating necessary medical treatments while ensuring continuity of care. Ideal candidates will have a Bachelor’s degree in the human services field and substantial experience in social services, particularly in the HIV/AIDS sector.
