Older Adult Intensive Case Manager# Older Adult Intensive Case ManagerDR STANLEY AND PEARL GOODMAN JFS OF BROWARD COUNTY INC - DAVIE, FL 33328## OverviewSalary Range$52,000.00 - $55,000.00 SalaryPosition TypeFull Time## Description**Department:** Case Management **Reports to:** Chief Operating and Program Officer **Job Title:** Older Adult Intensive Case Manager (In-Home Services) **Classification:** Professional, Full-Time/Exempt**Summary:** The Older Adult Intensive Case Manager (In-Home Services) provides high-touch, field-based case management and care coordination to older adults (typically age 60+) with complex medical, behavioral health, or social needs. This position involves frequent home visits to assess client well-being, coordinate services, and provide intensive support aimed at helping older adults age safely and independently in place.**Responsibilities:****Home-Based Assessments:** Conduct comprehensive psychosocial assessments in client homes to determine needs related to mental health, medical care, safety, housing, and daily functioning.**Care Planning:** Develop, implement, and monitor individualized care plans that address the unique needs of each client, with regular updates based on ongoing home visits.**Field-Based Support:** Provide consistent, face-to-face visits to older adults in their homes, offering emotional support, systems navigation, and linkage to services.**Service Coordination:** Facilitate referrals and coordinate with home health, primary care, mental health services, in-home support services (IHSS), meal programs, housing providers, and other community resources.**Crisis Intervention:** Respond to client crises in the home, including mental health decompensation, suspected elder abuse/neglect, eviction risk, or sudden loss of caregiver support. Management retains the discretion to add or change the duties of the position at any time.## Qualifications**Qualifications*** Bachelor’s degree in social work, Gerontology, Psychology, Nursing, or related field (master’s degree preferred).* Minimum 2 years of case management or direct service experience with older adults in a community or home-based setting.* Knowledge of aging-related conditions (e.g., dementia, mobility issues, caregiver burden).**Licenses/Certifications:*** Valid driver’s license, reliable vehicle, and current auto insurance (required for in-home visits).* CPR/First Aid certification preferred or willingness to obtain.**Skills:*** Ability to work independently in the field with vulnerable populations.* Strong interpersonal and cross-cultural communication skills.* Familiarity with mandated reporting laws (elder abuse, neglect, self-neglect).* Comfortably using mobile technology and electronic documentation systems.**Working Conditions:*** Majority of work is performed in client homes and community settings.* Exposure to varied home environments, including those with pets, clutter, or environmental hazards.* Must be comfortable working with clients who may have significant physical, behavioral, or cognitive challenges.* Occasional flexible hours to accommodate client needs or emergencies. #J-18808-Ljbffr Priority Dispatch Corp.
