RN Care Coordinator: In-Home Hospice Case Manager
Chapters Health System
Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate a Plan of Care with the Interdisciplinary Team, and provide clinical and palliative care to help patients remain at home as long as possible. The role requires an active RN license, BLS certification, and experience with EMR/EHR systems.
