Hospice of New Jersey is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team. You will provide hands-on hospice nursing care and coordinate services across the community with a compassionate approach. As part of our interdisciplinary team, you will educate patients and families, guide care plans, and ensure comfort and dignity at the end of life.
