LCSW Care Manager Psych - Evenings Full Time

LCSW Care Manager within our Psychiatric Department at Providence Little Company of Mary Medical Center in San Pedro, CA. This position is Full-Time and will work 8-hour Evening shifts (3pm - 11:30pm) to include weekend rotations. Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. • Master's degree in social work.
• Coursework/Training: All LCSWs must follow the guideline by Los Angeles

County Department of Mental Health for LPS designation and keep their designation in current status.

• Upon hire: California Clinical Social Worker License
• Within 18 months of hire: Designated as Behavioral Health Clinician to

perform 5150 evaluations * Within 1 year of hire: CPI certification (nonviolent crisis intervention training) * 2 years of Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting. * Computer experience. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
