GENERAL SUMMARY:

The Practice Transformation Project Specialist is the driving force behind high-impact transformation projects and performance reports, ensuring successful implementation of workflows, procedures and project goals that consistently achieve the Triple Aim—improved health outcomes, enhanced patient experience, and smarter per capita costs. You'll also be the vital bridge connecting post-acute facilities and agencies with the health system.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

• Collaborate with leadership and team members to champion standard improvement activities across post-acute providers, fueling network-wide performance gains.
• Lead the charge on transformation projects tied to payor initiatives and the triple aim, bringing energy and vision to every initiative.
• Drive project success by wielding project management tools to orchestrate collaborative planning, coordination and tracking of project activities.
• Dive into multiple data sources and information streams, craft compelling summaries and equip post-acute providers with the accurate information, tools and support they need to crush their performance improvement goals.
• Energize staff, community partners and physician practices through training and education on program requirements, practice obligations, resources, data collection tools, quality standards, measurement methodologies and report interpretation.
• Jump in to help prepare proposals, reports, updates and summaries for all assigned projects, hitting every deadline with excellence.
• Actively engage in sharing and learning opportunities related to improvement initiatives and similar transformation activities.
• Build and nurture strong working relationships across colleagues, leadership, committee and board members, providers, practice managers, community partners and staff—turning input and buy-in into collaborative momentum.
• Stay plugged into the broader healthcare landscape, actively seeking opportunities to collaborate and share resources, tools and materials that move the needle.

Qualifications

EDUCATION/EXPERIENCE REQUIRED:

• Bachelor's degree and at least three (3) years of hands-on experience in a health-related field.
• Solid project management experience under your belt—whether from a clinical network, provider organization, or similar healthcare setting.
• Demonstrated ability to communicate effectively (both verbally and in writing) and build genuine relationships with physicians, management, staff, external agencies, and patients/families.
• Computer savvy with word processing and spreadsheet software—think of it as your digital toolkit for success.
• Data detective skills: the ability to dig into performance data from multiple sources and turn insights into smart, actionable solutions.
• Master juggler status: proven ability to prioritize multiple competing priorities and adapt your approach for diverse customer populations.
• Project management training completed or willing to complete within six (6) months of hire—we'll help you level up!

PHYSICAL DEMANDS/WORKING CONDITIONS:

• Expect 80% local travel to post-acute care facilities throughout the market area.
• Comfort working in a dynamic healthcare environment with varying patient types, acuity levels, temperature conditions, and potential exposure to hazardous materials.
• Ability to navigate the normal hazards associated with a healthcare setting.
