Provide expert pain and symptom management focused on comfort and quality of life. Serve as the primary Hospice RN Case Manager for patients and families. Complete comprehensive nursing assessments and individualized care plans. Educate and support caregivers through every stage of the hospice journey. Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners . Provide care in patients' homes, assisted living communities, and skilled nursing facilities . Maintain accurate EMR documentation and participate in a weekend and on-call rotation . What You'll Bring Active Minnesota Registered Nurse (RN) license. Experience in hospice preferred , or a strong background in oncology, ICU, palliative care, home health, or other complex clinical settings. Strong clinical judgment and the ability to work independently. Exceptional communication and relationship-building skills. A passion for patient-centered, compassionate hospice care.
