Perform emergency nursing visits for symptom management, pain control, medication administration, and clinical assessment. Provide telephone support and education to patients, families, and caregivers. Collaborate with physicians and the interdisciplinary hospice team to coordinate care. Complete accurate and timely EMR documentation. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience preferred; nurses from home health, oncology, ICU, palliative care, or other end-of-life care settings are encouraged to apply. Excellent assessment and critical thinking skills. Ability to work independently while managing urgent clinical situations. Strong communication and compassionate patient care.
