Onboarding & Care Coordination Specialist
Commonwealth Care Alliance
Commonwealth Care Alliance is seeking an Onboarding Specialist to serve as the primary contact for members transitioning to a CCA/CareSource plan. You will reach out by phone, document existing services and new requests, and connect members with the care team. You will educate members on plan offerings, coordinate nurse visits, and use motivational interviewing to engage members.
