This position provides mental health assessment, counseling, treatment planning, crisis intervention, care coordination, and discharge planning services to individuals and families. The ideal candidate is passionate about improving the emotional well being of patients while working collaboratively within a multidisciplinary healthcare environment. The Clinical Therapist provides quality mental health services for the evaluation, treatment, and prevention of mental and emotional disorders in individuals, families, and groups. Utilizing evidence-based therapeutic interventions and sound clinical judgment, the therapist assists patients in achieving treatment goals and improving overall functioning while ensuring compliance with professional, ethical, and regulatory standards.**KEY RESPONSIBILITIES*** Conduct comprehensive psychosocial assessments and behavioral health evaluations.* Provide individual, family, and crisis intervention therapy services.* Develop, implement, and monitor individualized treatment plans with measurable goals and outcomes.* Coordinate care with physicians, treatment team members, patients, families, and community providers.* Maintain timely, accurate, and compliant Electronic Health Record (EHR) documentation.* Assist with discharge planning and coordination of aftercare services.* Follow up with patients and families regarding missed appointments and treatment continuity.* Obtain and maintain necessary releases, consents, and authorizations.* Participate in interdisciplinary treatment team meetings and communicate patient progress effectively.* Serve as a resource regarding community mental health services and referrals.* Respond appropriately to patient and family crises and provide consultation regarding complex cases.* Ensure compliance with Joint Commission, Medicare, state licensure, and hospital standards.* Maintain patient confidentiality and adhere to all HIPAA requirements.* Follow hospital safety, infection prevention, and risk management protocols.* Attend required meetings, trainings, and continuing education programs.* Support Fairfield Memorial Hospital's commitment to Service Excellence and patient-centered care.* Perform other duties as assigned.**COMPENSATION:** **$**55,120-$83,620 annually**SCHEDULE:** Day Shift**BENEFITS*** 403(b) Retirement Plan* PPO Health Insurance Plan* HDHP Health Insurance Plan w/ Health Savings Account* Dental Insurance Plan* Vision Insurance Plan* Short and Long Term Disability Insurance* Basic and Voluntary Life Insurance Plans* Paid Time Off (Vacation/Sick/Illinois Paid Leave/Holiday)* Flexible Work Schedules* Professional Development Opportunities**WHAT FMH HAS TO OFFER** Fairfield Memorial Hospital is a mission focused critical access hospital dedicated to providing exceptional care to the patients and families we serve, with a strong commitment to improving the health and well being of our community through compassionate service, clinical excellence, and a supportive culture that empowers employees to do their best work. We proudly uphold Equal Opportunity Employment practices and welcome applicants of all backgrounds without regard to race, color, religion, sex, national origin, sexual orientation, gender identity, disability, or veteran status.## Qualifications**EDUCATION/QUALIFICATIONS*** **Required:** + Master's degree in Social Work, Counseling, Psychology, or a related behavioral health field from an accredited institution. + Current and unrestricted Illinois licensure as one of the following: Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC).* **Preferred:**Bachelor of Science in Nursing; Certification in Case Management (CCM, ACM, or related certification).**EXPERIENCE*** **Preferred:** Two (2) or more years of clinical experience in behavioral health, outpatient therapy, hospital-based mental health, or community mental health settings.**SKILLS*** Mental health assessment and psychosocial evaluation* Individual, family, and crisis intervention therapy* Treatment planning and care coordination* Clinical documentation and EHR proficiency* Knowledge of behavioral health best practices and ethical standards* Strong communication, interpersonal, and active listening skills* Critical thinking and clinical decision-making abilities* Ability to work independently and collaboratively within a multidisciplinary team* Knowledge of community resources and referral coordination* Commitment to patient-centered care, professionalism, and confidentiality* Strong organizational and time management skills* Cultural competency and compassionate patient care
