Responsibilities: Provide direct patient care to residents, including administering medications, performing vital signs, and providing wound care Conduct routine checks on residents' conditions and report any changes or concerns to the Director of Nursing Assisting with admissions, discharges, and transfers, ensuring a seamless transition for our residents Collaborate with the interdisciplinary team to develop and implement individualized care plans Maintain accurate and up-to-date medical records, ensuring compliance with regulatory requirements Participate in daily report and handovers to ensure continuity of care $30-36/hour About the Role: As an Evening Shift LPN, you will be responsible for providing high-quality care to our residents while working closely with our dedicated team of healthcare professionals.
